Terms and conditions

- I understand that I and/or my child do not own the copyright or have any rights of ownership or other claims over copy, photographs, video footage or audio recordings, and information about me or the child for whom I have parental responsibility (but this does not interfere with rights under GDPR).
- I understand that any information about myself and/or the child for whom I have parental responsibility will only be used for the purposes stated in the consent form and as set out below.
- By consenting to share your story, which for these purposes your story includes that of your child, you agree to have your story information, including your contact details and health condition, stored on our secured Heart Story library which is only directly accessible by BHF Heart Stories team.
- Your story and interview notes will be shared with BHF’s Clinical team to check for any medical inaccuracies/ambiguities, and where any inaccuracies are found will be amended.
- Permission for BHF to use and share your story applies for five years from the date of submission of the consent form, without further contact from BHF.
- During the 5-year consent period, BHF may seek additional content or permissions regarding your story.
- BHF may contact you before the expiration of the 5-year consent period to ask if you wish to reconsent to share your story.
- You (or your child when they reach 18) may withdraw part or full consent for any of the story or information or specific uses at any time during the 5-year period by contacting BHF on 0300 330 3322 or at [email protected]. Withdrawal of consent means we will stop using that information/story or for that purpose in future.
- Any previously published materials that include your information and/or story that has been issued before any withdrawal of consent or expiration of the consent date cannot be recalled but we can take them off our live website.
- Any protected characteristic information given such as gender, ethnicity, disability, sexuality, religion, etc will be held by the BHF Heart Stories team only and not shared internally or externally without express permission. Where this may be evident from any photo, video or your story you allow it to be used.
- By consent to use your story and/or image, or that of your child, by completing the online consent form, you agree that your information will be stored and used by BHF, and its partner organisations, in printed, online or electronic publications which may be shared publicly.
- Examples of materials or outlets your story and/or image, or that of your child, may be included in, but not limited to, are:
a. Posters, leaflets, brochures
b. Printed and digital advertisements
c. Internal and external emails
d. E-newsletters
e. Blogs and articles
f. Web pages and podcasts
g. Webinars
h. Social media posts
i. Internal and external presentations and/or speeches
j. Research or policy reports
k. Press releases or other materials - BHF will ask for your express permission to share your story and details, or that of your child, before broadcast or publication in third party outlets such as TV, radio, newspaper, podcast interviews, etc.
- BHF is not liable or responsible to you for any use, mistakes or errors made by any third-party organisations after publication of a story externally or be held liable for use of story information or imagery by a third party.
- BHF reserve the right to withdraw a story being used, or an offer of a story to be used, for internal and/or external purposes at any time for any reason without notice.
