del Nido versus St. Thomas’ blood cardioplegia: a multi-centre randomised controlled trial in children undergoing cardiac surgery

During open heart surgery, a fluid called cardioplegia solution is flowed through the heart to temporarily stop it from beating. This allows the surgeon to operate safely without the interference of heart motion. Blood flow to and from the heart is also bypassed. Restoring blood flow can cause additional damage to the heart muscle – known as ischaemia-reperfusion injury (IRI). In children undergoing heart surgery, IRI may affect recovery. There are different types of cardioplegia solution available, which work in different ways. St Thomas’ solution is commonly used in adults and children undergoing open heart surgery in the UK, while del Nido solution is commonly used in children in the US. There is evidence that del Nido solution may be better at reducing the effects of IRI in young heart muscle. Mr Drury will lead a trial to compare how well these two solutions can protect the heart in children undergoing heart surgery. The trial aims to recruit 220 children undergoing surgery that requires the heart to be stopped. Half will be randomly assigned to receive del Nido solution, and half will receive St Thomas’ solution. The researchers will compare the two groups by assessing how well the children recover from surgery and determining the extent of any heart damage. This pilot trial could provide the basis for carrying out a larger trial to provide an answer to which solution is best for this type of surgery and could result in heart surgery becoming even safer for children around the world.
