Web Gift Aid Declaration
As part of the Gift Aid scheme it is now possible for Charities to reclaim the tax you pay on your donation if you are a basic rate tax payer.

If you tick the first box this means that for every £1 you give the BHF can claim an extra 28p from the tax man. And it won't cost you a penny extra.

Yes I am a UK taxpayer and would like the BHF to reclaim the tax on any of the donations I have made in the last 6 years and any future donations I may make. *

Date: 20 July 2008

Notes
  • In order for the BHF to reclaim the tax on donations, the person making the donation must pay an amount of income tax or capital gains tax at least equal to the tax we reclaim from the Inland Revenue (currently 28p for every £1 you give)
  • For Gift Aid donations, please ensure that cheques from a joint account are signed by the tax payer
  • You may cancel this declaration at any time (and must do so if you cease to be a UK taxpayer) by notifying the BHF at the address given above)
  • The BHF will use tax reclaimed on your donations in the most effective way to fulfil its objectives
No I am a non-taxpayer.
Your payment details will be processed securely using WorldPay plc

Thank you very much for your support

What happens to your personal information
The British Heart Foundation (BHF) values your support. We will use information provided by you for administration and marketing purposes. We may contact you by post or occasionally by phone or e-mail and may include news and information on BHF's charitable work, (e.g. how your money is spent, heart health information), BHF events, and related products and services from our subsidiary companies such as Christmas gift catalogues.

Please tick the box if you do NOT want to hear from the BHF at all.

Occasionally the BHF may pass on your details to carefully selected third party organisations we are working with, for them to send you information on their events, products and services.

Please tick the box if you do NOT want your details passed on in this way.
