Pharmacists

Pharmacists work alongside heart failure nurses to:

• run community and hospital based heart failure clinics
• carry out home visits
• case manage patients who are frequently admitted to hospital
• run a dedicated prescribing service for heart failure patients
• provide education to other healthcare professionals on heart failure medication related issues.
