Cardiac Liaison Nurses
CLNs provide a seamless pathway of care for
patients following a heart attack or cardiac surgery.
This includes:
- visiting patients in their own home during the immediate period
post-discharge from hospital to provide reassurance and advice to
them and their carers
- linking with the community healthcare team to provide education
and advice on the care of patients with heart disease and their
long term follow-up
- providing specialist lifestyle advice such as smoking
cessation, weight management, exercise and healthy eating
- preparing patients and carers for further investigations and
treatment following their heart attack
- educating other healthcare professionals in primary and
secondary care to make sure the latest evidence is available to
patients to prevent further heart disease.
