Hearty Lives West Norfolk

Walking groupWe are investing £100,000 over three years to reduce health inequalities and encourage people to adopt healthier lifestyles in some of the most deprived areas of West Norfolk.

In an innovative partnership arrangement, GPs from the surgeries in the West Norfolk Clinical Commissioning Group area will refer patients to the local Health Trainer Team, who in turn will support patients to make healthier life choices and access a wide range of healthy lifestyle services.

Our Aim

  • make a positive impact on the health and lifestyles of people living in some of the most deprived wards in West Norfolk – particularly around King’s Lynn, Downham Market and Swaffham
  • pioneer a Lifestyle Referral Pathway with professionals working in primary care, by encouraging clinicians and nurses to refer patients confidently on to local health improvement services
  • provide and promote local Public Health leadership, by co-ordinating and supporting a wide range of lifestyle interventions, and building this into the long term aims of the West Norfolk Clinical Commissioning Group
