A key part of our work is empowering people by providing the information they need to reduce their own heart risk.
That message applies to people of all ages and backgrounds - but some groups are harder to reach than others.
'Reaching out'
How do you get school children interested in heart health? Or connect with teenagers who spend more and more of their time using the web and social media?
Our aim this year has been to reach out to these audiences in even more imaginative ways.
Yoobot has certainly done that. It's part of our Food4Thought initiative, designed to encourage children to think about the long-term consequences of their food and lifestyle choices.
Exciting new technology allows them to create an online mini-version of themselves at yoobot.co.uk, which they can then experiment with to see the effects of different diets and forms of exercise. Since the launch last November, over one million Yoobots have been created, making it one of the 'must-have' free games in the run-up to Christmas.
Meanwhile, we've been attracting thousands of visitors to our Facebook page, podcasts and YouTube channel, where one heart health video has been viewed over 7,000 times.
We're always conscious of the need to move with the times and take advantage of the best that new media channels have to offer.
'Positive feedback'
We're growing our presence on Facebook, YouTube and Twitter and attracting a lot of positive feedback.
Meanwhile, Yoobot remains a central part of our Food4Thought campaign as it enters its fifth year.
